Forms for filing a Business Certificate may be purchased at the County Clerk's Office at $3.00 each. Completed DBA Certificates are filed in the Receiving Department at the County Clerk's Office. For sole proprietorship, partnerships, amending or discontinuing of DBA’S:

Forms must be completed, Signed and Notarized when presented for filing.

The following forms may be obtained either through an attorney (having him/her take care of it for you) or at the County Clerks Office:

BUSINESS CERTIFICATE (or DBA) –used for sole proprietor or individual. (Blumberg Form X-201)

BUSINESS CERTIFICATE FOR PARTNERS – Used when business consists of one or more partners. (Blumberg Form X-74)

AMENDED BUSINESS CERTIFICATE - Used to amend information on original business certificate filed, such as change of address or adding or dropping a partner. (Blumberg Form T-224)

DISCONTINUANCE – Used when discontinuing a business (sole or partners) that is on file. (Individual T-341) (Partnership T-176)

5.00 Certified Copy (copies ONLY for certification)

NO FEE to file a Discontinuance

We will accept check, cash or money order for payment of fees.

Corporations, Limited Partnerships, LLC’s or LLP’s must be filed with the Department of State, NYS DIVISION OF CORPORATIONS (518) 473-2492. These papers cannot be filed at the County Clerk’s Office
